For devices to qualify for Medicare and most other insurance plans, all statutory and regulatory requirements must be met.  Medicare specifically requires a detailed physician evaluation of the patient.  Please share the PDFs below with your physician to ensure the chart notes meet compliance standards.  Note that Medicare does not accept a letter as part of the medical record.
